To complete an electronic application/supplement and email directly to DefensePro:

1. Choose the appropriate form from the list at left
2. Open the Word document and save to your desktop
3. Complete the form at your convenience
4. E-mail completed form to submissions@defenseproliability.com

To print form, complete and fax/mail to DefensePro:

1. Choose the appropriate form from the list at left
2. Open the PDF file
3. Print the form
4. Complete the form and:
   a. Fax to 913-652-7599 OR
   b. Mail to DefensePro at the address shown below
